Your Labs Look Fine and You Still Feel Flat. Prolactin May Be the Reason.

You run the company, the practice, the household — sometimes all three. Your labs came back “normal.” Testosterone sits in range. Thyroid sits in range. And still the drive is gone, the training feels heavier than it should, and the ambition that used to be automatic now takes a decision. High prolactin symptoms are quiet, non-specific, and easy to write off as stress or age.

The Pituitary Signal That Overrides Everything Downstream

Prolactin is produced by lactotroph cells in the anterior pituitary. Most people associate it with lactation and stop there. It is present and active in men and in non-pregnant women, and it influences far more than one function.

Prolactin is unusual among hormones. It is held down rather than pushed up. Dopamine released from the hypothalamus continuously suppresses it. Anything that lowers dopamine tone, or that stimulates the pituitary directly, lets prolactin drift upward.

That drift matters because prolactin sits upstream. Elevated prolactin blunts the pulsatile release of GnRH from the hypothalamus. Fewer GnRH pulses mean less LH and FSH from the pituitary. Less LH and FSH means less testicular testosterone output in men and disrupted ovulation in women.

So the sequence runs backward from how most people investigate it. You feel the downstream effect — flat drive, poor recovery, mood that has lost its edge — while the downstream number still reads acceptable. By the time testosterone or progesterone looks clearly abnormal, the upstream signal has been off for a long time.

Clinical Note Most conventional labs flag men only above roughly 20 ng/mL and women only above roughly 25. Functionally, a symptomatic man sitting at 15 and a symptomatic woman sitting at 22 are worth a second look. The reverse is also true: prolactin can read falsely high because of macroprolactin, a biologically inactive bound form. An elevated number in someone with no symptoms should be screened for macroprolactin before anyone builds a plan around it.

What Actually Pushes Prolactin Up

Prolactin has more common drivers than most people expect, and the common ones are far more likely than the rare one. Working through them in order is what separates a useful result from an alarming one.

  • Sleep debt and sustained stress. Prolactin is acutely stress-responsive. A short night, a hard week, or even an anxious blood draw can move the number.
  • Medications. SSRIs, antipsychotics, metoclopramide and some reflux drugs, opioids, and several blood pressure medications all raise prolactin as a known effect.
  • Low thyroid output. When TSH rises, TRH rises with it — and TRH stimulates prolactin release directly. A thyroid pattern and a prolactin pattern often travel together.
  • Chest-wall stimulation. High-volume pressing, a tight heart-rate strap, or a loaded ruck can transiently raise it.
  • A prolactinoma. A benign pituitary adenoma. Uncommon, but real — and the reason a persistently high result belongs in front of a physician for imaging rather than in a supplement plan.
A hormone panel that stops at testosterone answers the wrong question. The signal that sets the ceiling sits one level up.

Who Should Actually Measure It

Prolactin is not a universal screening marker. It earns its place on a panel when the pattern points at it.

  • Men with low drive and mid-range testosterone that will not respond to standard optimization
  • Women with irregular cycles, unexplained difficulty conceiving, or unexpected nipple discharge
  • Anyone on an SSRI, an antipsychotic, or a long-term reflux medication
  • High performers carrying chronic sleep debt alongside heavy training volume
  • Anyone already showing a hypothyroid pattern on labs
  • Anyone with new headaches or changes in peripheral vision — that pattern goes to a physician first, not to us
